Steno Diabetes Center Copenhagen (SDCC) is a public hospital in the Capital Region of Denmark. The centre was established through a public-private partnership between the Capital Region of Denmark and the Novo Nordisk Foundation and integrates specialised care, research, innovation and education to advance the prevention and treatment of diabetes and related chronic conditions. As Northern Europe’s largest diabetes centre, SDCC serves as a leading example of how these elements can be combined to create more coordinated and patient-centred healthcare.

This visit provides insight into how an integrated, multidisciplinary healthcare model can support prevention, treatment and long-term management of chronic diseases. Delegates will gain first-hand insight into how patient pathways are designed and coordinated across sectors, and how digital tools and data-driven approaches support prevention, equitable access to care and better health outcomes. The visit will also explore the close connection between obesity and type 2 diabetes, highlighting how early intervention can reduce the burden of disease throughout life.

Participants will visit clinical and innovation environments and learn how new knowledge is translated into practice through collaboration between healthcare professionals, researchers and public partners. The programme is designed not only to share Danish experiences, but also to create opportunities for dialogue and exchange on shared challenges related to chronic disease management, prevention and sustainable healthcare delivery.
